EECP works on the principle of generating a “counter pulse” towards the patient's heart using “Pulsatile Pneumatic Compressions”. The EECP machine consists of a compressor that uses compressed air to inflate and deflate the cuffs, first around calves then the thighs, and finally the hips cuff). The pressure moves the blood from the lower limbs toward the patient’s heart. Each pressure wave is timed to the patient’s heartbeat to reach the heart when it is relaxing and the blood flow to the heart arteries is maximized. When the patient's heart pumps again all the cuffs deflate simultaneously. This enables the patient's heart to pump into empty vessels, thus significantly reducing its workload and improves the ejection.

EECP Therapy improves Cardiac Function by improving the perfusion of the Coronary Vasculature (Blood Supply to the Heart itself). It opens up the new collaterals and hence improves circulation and perfusion. It improves the micro-circulation of the patient's heart (including Distal perfusion) and the Ejection capacity of the patient's heart. EECP Treatment is not for everyone. Here are some examples, who should not get EECP Treatments; Patients with Uncontrolled Hypertension (Control the Blood Pressure first to the acceptable range), patients with Severe Valvular Disease. Moderate to severe AR (Aortic Regurgitation) is absolute “NO” for EECP Treatment. Other Contraindications for EECP Treatment include Aortic Dissection, Cardiac Arrhythmias, Deep Vein Thrombosis (DVT) Uncontrolled Congestive Heart failure grade 3 & 4, Aortic Aneurysm, Septal Defects (ASD, VSD), Pregnant Woman, Severe Peripheral Arterial Disease, Clotting Disorders, etc.

Even though it has been claimed by several clinics or practitioners the Effect of EECP Therapy is still not proven in Uncontrolled Diabetes Mellitus, Diabetes Nephropathy, Senile Dementia, memory enhancement, Vascular Headaches like Migraine; There is a need for more trials and studies to justify these claims. EECP Therapy may presumably help with Erectile Dysfunction, Chronic Fatigue Syndrome, improve Energy Levels; still more trials and studies needed. Patients undergoing EECP Procedure may rarely report, muscle pain, bruises due to the Compression of the cuffs, headache due to increased blood flow to the Brain, giddiness due to lowering of Blood Pressure. Fracture is “never” reported but may happen if the patient is having very severe Osteoporosis. In extremely rare conditions EECP procedure may cause embolism to the lungs or aggravate CHF / Heart Failure because of increased blood flow to the failing heart.

EECP therapy timeline after a Cardiovascular procedure like CABG or PTCA are suggested as follows: EECP Treatment may be done after 1 week if the Coronary angiography was done through Radial Route (Hand). EECP Treatment may be done after 2 weeks if the Coronary angiography was done through the groin (Femoral). EECP Treatment may be done 6-8 weeks after PTCA (Stenting/Angioplasty) 12 weeks after CABG (Bypass Surgery)

Before any patient can start EECP Natural Bypass Therapy or Treatment a Complete Screening and thorough Investigations are needed including Complete Medical History, ECG, Pulse Oximetry, 6 minutes’ walk test, Stress Test, 2-D Echocardiography, Color Doppler of both the lower limbs, Ultrasound of Abdomen and Pelvis, Dexa Scan (in fragile patients), Coronary Angiography (if not done previously). It is also preferable to do a Diabetes Screening and Routine Laboratory Investigations before EECP treatment. Other investigations are based upon medical & Surgical History provided by the patient.
